The following restaurants received ratings of 98 percent or above during recent inspections by the Forsyth County Health Department:
• Four Star Cafe, 299 Jonestown Road, 100.5 percent (with two bonus points).
• Panera, 105 Hanes Square Circle, 99.5 percent.
• Wendy's, 1429 Lewisville-Clemmons Road, Clemmons, 98 percent (with two bonus points).
The following received ratings of 89 percent or below:
• Mr. Waffle, 649 Peters Creek Parkway, 88.5 percent. The inspector noted problems with cleanliness, food temperatures, drying dishes and damaged equipment.
• Wendy's, 3182 Peters Creek Parkway, 87 percent. The inspector noted problems with cleanliness, flies, drying dishes and damaged equipment.
• O'Charley's, 300 Hanes Mill Road, 86 percent. The inspector noted problems with cleanliness, drying dishes and dirty and damaged equipment.
